No refund for cancellations will be entertained after September 15, 2023. Substitutions will be allowed only until September 25, 2023. The class roster will be closed on September 25, 2023.

NOTE: We understand an applicant may not be the cardholder or authorized purchaser at his/her agency. If there will be a delay paying your invoice because of this obstacle, simply provide a brief note in the comments section at the end of the form.
